Preliminary Online Application for Dog

The PetConnect adoption process includes a consultation with a PetConnect representative and is designed to help you find the dog most compatible with your family. We are committed to placing each dog in the right home for that particular dog. We take time to know each dog and can give you detailed information about his/her behavior and personality.

In order to be considered as an adopter you must:

Be 21 years of age or older
Have identification showing your current address
Have the knowledge and consent of your landlord and/or all people residing with you
Be able and willing to spend the time and money necessary to provide training, medical treatment and proper care for a pet
Be willing for a PetConnect representative come to see where the dog will be living

Please read the Pet Connect Animal Adoption Agreement before applying to adopt any pet.
